Job Title: Home-Based Locality Social Worker (Adults Services)

About the Role:

A London-based Local Authority is seeking a skilled and experienced Social Worker to join their Adults Locality Service on a locum basis. This role offers the unique opportunity to work predominantly from home, with a focus on delivering high-quality care and support to adults in the community. You will play a crucial role in ensuring service users receive appropriate and tailored care packages, contributing to their overall well-being and independence.

Key Responsibilities:

Care Act 2014 Compliance: Undertake assessments and reviews in accordance with the Care Act 2014, ensuring that service users are receiving care packages that are both appropriate and effective in meeting their individual needs.

Person-Centred Planning: Develop and implement personalised care and support plans, working collaboratively with service users, their families, and other professionals to ensure that the care provided is aligned with the service user's goals and preferences.

Multi-Disciplinary Collaboration: Work closely with Occupational Therapists (OTs), Housing, Employment, and Health Services to deliver holistic care that addresses not only the physical needs of service users but also their social, environmental, and psychological needs.

Autonomy and Flexibility: Enjoy a high level of autonomy in your role, as you will be primarily home-based with the flexibility to manage your own schedule. Field visits are required to maintain direct contact with service users, ensuring that the care provided continues to meet their evolving needs.

Risk Management: Conduct thorough risk assessments to identify potential safeguarding concerns and take appropriate actions in line with the Safeguarding Adults framework. Your role will be crucial in protecting vulnerable adults and promoting their safety and well-being.
